To apply online you will need Adobe Acrobat to open the file. If you do not have Adobe please click the icon below. Click on the application link below. Once you pull up the application, you will have the ability to actually type in the application. Just place your mouse over each section that you will need to fill out and click, once you see the blinking cursor you can start to type. You can also use the TAB button on your keyboard to move you to the next blank. Once you have the application filled out, just click the print icon and your application will be printed with your information. Or, if you choose to, you may just print the application and fill it out by hand.

Please enclose the membership fee/renewal fee of $100.00 (or pay online by clicking the add to cart button next to the application link listed below), letter of appointment by your department, and letter of appointment by your ecclesiastical body with your application and send to: Federation of Fire Chaplains, 185 County Road 1602 Clifton, TX 76634. Once your application is received, you will be contacted by phone regarding your membership.
